The Supplemental Nutrition Assistance Program (SNAP) provides supplemental assistance to help low-income individuals and families purchase food.

SNAP benefits are based on household composition, income and countable expenses for all household members. All new applicants will be required to complete a phone interview and may be required to provide verification of income or expenses. Benefits are issued through an Electronic Benefit Transfer (EBT) card that is used like an ATM card and accepted at most grocery stores.

How do I apply for SNAP? 

  • Access the Colorado PEAK website to apply online
  • Call 970-241-8480 to request an application to be mailed to you
  • Print a paper application from the Department of Health (SNAP) website.
  • Call our SNAP outreach partner, Hunger Free Colorado, at 855-855-4626. They can help you complete an application over the phone.
  • If you would like to request a new EBT card or a pin change, you may call 1-888-328-2656 to report your card lost, stolen or broken and request a replacement card be mailed to you.
  • Call the SNAP hotline directly at 1-800-816-4451 to check on SNAP benefits.
